Enquiry Form - Disabled Facilities Grant

Enquiries for Disabled Facilities Grant are directed through Wiltshire County Council’s CareFirst project. We will pass your details to the CareFirst team who will contact you directly. They may ask for an assessment of your needs to be carried out by an Occupational Therapist, who may recommend adaptations to your home to be paid for through Disabled Facilities Grant.
